EDAM Waiver

Every person who attends an EDAM class, workshop or JAM acknowledges having read these waivers and agrees to release and save harmless EDAM Performing Arts Society and its directors, employees and agents from all claims, demands, and liabilities associated with or issuing out of their use of the premises, facilities, instruction and activities of EDAM.

Acknowledgment and assumption of risk: I acknowledge that I am aware of the nature of dance training, that there is an element of personal risk, damage or serious injury in these activities and willingly agree to assume responsibility for these risks as a condition of registering for and participating in any EDAM classes, workshops, or JAMs.

Indemnification and release: I represent that I have no physical condition that would prohibit my participation in these classes and that I, on behalf of myself, my heirs, legatees and assigns, agree to indemnify, save and hold harmless EDAM Performing Arts Society or any of their agents, representatives, employees or assigns for my health, safety and any injury and/or disability arising out of or resulting from EDAM classes or workshops.

EDAM is not responsible for any lost or stolen valuables.

The Social COVID Contract

I acknowledge that I am aware of certain risks of participating in/organizing an event due to the COVID-19 pandemic, and that I am choosing to enter and use EDAM’s studio-theatre under the following understanding:

MY UNDERSTANDING: I understand that by leaving my home, I am exposing myself to a greater risk of becoming infected with COVID-19, or of infecting other people. I understand that it is my obligation to stay away if I have symptoms of have been diagnosed with COVID-19. I understand there are specific protocols that EDAM is asking me to abide by and I agree to do so. I understand that it is recommended to carry a daily contact log which may if so required facilitate contact tracing should anyone fall ill.

MY PROMISE: In order to do my part to not spread this virus, I agree to not enter the venue and to return home if (a) I have a temperature above 99.7 F or above 37.6 C, or symptoms such as coughing and sneezing (b) if EDAM determines that I may not safely enter the venue and/or (c) there is reason to believe that I carry COVID-19 despite a current absence of symptoms. I will abide by all recommendations as per the public health authority; and do my part in minimizing in other ways the risks involved in spreading the virus. I will notify EDAM if I have travelled outside of the province in the past 14 days, and I will stay away from the building until it is resolved that I am healthy. 

MY EXPECTATION: By entering the venue, I am not assuming all risk of contracting COVID-19, only the risk that cannot be avoided despite EDAM’s efforts to maintain reasonable health and safety under current circumstances.
